Prise en charge de l'insomnie chronique primaire par les médecins généralistes ambulatoires de l'arc alpin‎ : exploration des difficultés ressenties

Abstract : Objective: To study the difficulties encountered by GPs managing patients with chronic primary insomnia. Method: A qualitative study involving volunteer GPs in French Alps. A random sampling was chosen for wide variability. Interviews were recorded by an independent trained interviewer at the GP surgery between January and August 2012 using a semi-structured interview-grill. Interviews were exhaustively transcribed and a thematic analysis carried out by two researchers, thereby assuring triangulating researchers. Interviews were run until data saturation. Results: 13 GPs from varied backgrounds were interviewed and certain difficulties in diagnosis and management found. Often insomnia was not the motive for consultation, but mentioned incidentally at the end of the consultation when seeking renewal of medical prescriptions. This was perceived as a difficulty in management because of time and availability constraints since insomnia seemed to be considered as commonplace and not considered as serious thereby leading to underdiagnosis. During the consultation the doctors felt under pressure to renew prescription of hypnotics because of time constraints, or lack of motivation to resist patient pressure, and a feeling of difficulty in convincing patients to modify lifestyle. Drug withdrawal effects were also considered as a problem, especially depending on previous duration of treatment, levels of dependence, type of patient, and fear of withdrawal effects. Doctors were dissatisfied and in difficulty in relation to withdrawal effects. Conclusion: GPs have difficulty at all points of management of patients with insomnia with a feeling of lack of quality in patient management.
