État des lieux des connaissances des consultants au cabinet de soins primaires sur les principales questions de santé publique‎

Abstract : Our patients receive a lot of medical information and prevention advice through medical staff, the media and the Internet. How much do they retain? We designed a multicenter observational study to assess the knowledge of primary care patients on a set of key public health issues identified by the 2004 French Public Health Act. Three hundred multiple choice questionnaires were distributed in the waiting room of seven general practitioners in the French department of Isère. 280 patients between 18 and 88 years were included (median age 43 years) with a majority of women (68,1%). Several gaps in awareness and knowledge are evident in our study in the following areas: classes of analgesics and their dosage, the appropriate use of antibiotics, the need for iodine supplements, as well as failure to recognize the signs of stroke or melanoma. Knowledge well memorized by patients almost always corresponded to subjects well covered by major advertising campaigns in the media: the frequency of breast and cervical cancer screening, the frequency of tetanus vaccine booster, the damage caused by smoking, as well as the need to limit salt intake and less clearly in our study, cardiovascular risk factors and their prevention, recommendations concerning alcohol and the risks of HIV transmission. In multivariate analysis, only gender and study level were significantly associated with better knowledge. The study confirms that significant effort is still required to promote good health as it may have an impact on social inequalities in health and there is a strong demand from our patients.
