Contraintes à l'origine de la souffrance des internes en médecine‎ : analyse par entretiens semi-dirigés

Abstract : BACKGROUND: The findings of the caregivers' sufferance are being generalized and medical residents are no exception. In France, around 40% of them suffer from burnout syndrome. The elaboration of a prevention program seemed vital, but one could not avoid a thorough knowledge of the causes of sufferance. OBJECTIVE: Exploration of the commitments at the root of the medical residents' sufferance. METHOD: Qualitative study by semi-structured interviews of 12 general practice residents of Grenoble. The sampling was designed to search maximal variation. A thematic analysis of the verbatim was conducted by triangulation of the searchers, after full written transcription of the interviews. RESULTS: The results gather commitments which can be ranked at three levels: individual, interindividual and organizational. Some of the themes have already been described in past studies: the requirements of the medical studies, the relationship problems with patients and in-between caregivers, the lack of recognition, the responsibility in medical decisions, the psychological and emotional fatigue, and also some aspects of personality which make residents more sensitive commitments. To the contrary, some themes of sufferance are unexpected: the role of residents in society, the design of internships and the commitments to their practice. Finally, some commitments have been expressed in a strong manner: the concessions undergone on personal life (family life and friendships, leisure, health) and, within the studies theme, the trivialization of commitments experienced during medical training, disturbing the expression of sufferance. CONCLUSION: This study thoroughly apprehends the causes of residents'. It is now necessary to set up adequate awareness and support programs. In the meantime, efforts must be continued to improve their working conditions.
