Arthroplastie par prothèse totale trapézo-métacarpienne Roseland® dans la rhizarthrose essentielle : revue de 64 prothèses avec un recul supérieur à dix ans

Abstract : The Roseland® is a trapeziometacarpal prosthesis (TMP) for the surgical treatment of rhizarthrosis. Its short-term results are well known, but its long-term results are still controversial. The aim of our study is to assess the functional, clinical and radiological results of 64 prostheses in 51 patients, with a follow-up of over 10 years. After 12.5 years, 91% of the prostheses stay in place with a result always considered as "satisfying" to "very satisfying". The 3/4 of them are able to resume or increase their preoperative manual activity. The pain is absent or occasional in 91% of the cases. Amplitudes and force are satisfying according to the age of patients. Radiological axial sinking and periprostheic osteolysis are very frequent, with no effect on the functional results. Complications affect 25% of the studied prostheses, trapezial fractures being the first cause (9%). Eight sugical re-interventions have been undertaken (13%), six of them consisting in prosthetic removal / trapeziectomies (9%). Removals are due to multiple causes (loosening, trapezial fracture, prosthetic dislocation, metallosis, symptomatic peri-trapezial arthrosis). Our results concerning the mid and long term clinical results and survival are superior to the ones reported in the litterature about the other commercialized TMPs : the Roseland® prosthesis is therefore the best prosthesis currently on the market.
