Gestion en médecine générale des nouveaux anticoagulants oraux lors de chirurgie ambulatoire ou gestes techniques (réalisation d'une fiche de synthèse)

Abstract : In 2008, two Novel Oral AntiCoagulants (NOACs), the dabigatran etexilate and the rivaroxaban, were commercialised. They allow patients to undergo invasive procedure or even surgery. Here, we produce a summary and a practical guide that allow NOACs management during low hemorrhagic risk procedure. To this purpose, we produce a literature review on the current knowledge on NOACs. We also define the low hemorrhagic risk procedure and the assessment of a thromboembolic risk before the procedure. Our results show that very few studies connect NOACs with specific procedures. We thus rely on scientific societies recommendations and VKAs guidelines. In conclusion, for procedures with a low hemorrhagic risk and a moderate thromboembolic risk, we propose a therapeutic window where NOACs medication is stopped 24 hours before and until 24 hours after the procedure.
