Analyse qualitative de la prescription différée d'antibiotiques par des médecins généralistes‎ : étude dans les infections respiratoires hautes et basses (en dehors de la pneumopathie) de l'enfant de plus de 2 ans et de l'adulte sain

Abstract : Background: Despite substantial efforts, France remains a strong consumer of antibiotics. Some countries apply delayed prescription in order to rationalize the antibiotics in the treatment of respiratory infections of healthy patients. Few studies are available in France. Objective: We wanted to establish an overview of the practice: what are the pathologies, the influencing factors and the limits, the prescribing method and the general practitioners opinion. Methods: We made a qualitative study based on 22 individual interviews together with 2 focus groups of both Savoie departments general practitioners. Results: General practitioners stated that they sometimes use this practice. Child acute otitis media and adult's beginning sinusitis are the main pathologies for which delayed prescription has been used. The influencing clinical factors were medical history and superinfection, a long lasting evolution or on the contrary an early consultation. Patient understanding and a relationship of trust appeared to be essential. In case of organisational constraints, the practice was reassuring to the patient and the general practitioner, where a doubt in diagnostic existed, despite the absence of formal indication. General practitioners were conscious of the risks and their opinion on the practice differed. The prescribing methods and the recommendations were varying. Conclusion: General practitioners practice delayed antibiotics prescription in complex situations in order to avoid an immediate prescription. This strategy is not without risks. In order to better identify it, downstream studies are necessary.
