Prévalence, caractéristiques et regard des soignants sur les troubles des conduites élémentaires en milieu institutionnel gériatrique‎ : alimentation, sommeil et sexualité

Abstract : Frail elderly residents live in close proximity in Nursing Homes, frequently with multiple illnesses and showing signs of abnormal behavior linked to dementia. This cross-cutting epidemiologic study was carried out in geriatric institutions in Isere and Haute-Savoie between March and July 2013. The objective was to describe the extent and the characteristics of elementary behavioral problems, and the rate of exposure and related problems of care encountered by medical staff and general practitioners. Seventeen nursing homes were studied, and 1334 residents included. Five hundred and sixty six (42,43%) residents showed signs of abnormal behavior: 271 (20,31%) with feeding difficulties, 227 (17,02%) with sleep disturbances, 184 (13,79%) with inappropriate sexual behavior. The most frequent problems are refusing food (77,78%) for feeding difficulties, wandering about (50,50%) for sleep disturbances and explicit sexual comments or groping (56,36%) for inappropriate sexual behavior. The exposure rate for nursing staff is 100% for feeding difficulties, and 89,83% for sexual behavior. The main concerns felt by nursing staff are the fear of weight loss for feeding difficulties (53,70%) and embarassment (67,92%) for sexual behavior. Seventy five per cent of general practitioners claim they find it difficult to manage these issues. Elementary behavioral disturbances are frequent and polymorphous, sometimes taken badly by nursing staff. There is no agreed definition for these troubles and this study identifies a lack of training for nursing staff and GPs.
