Le médecin généraliste et la douleur ressentie par les patients diabétiques lors des autosurveillances glycémiques et des injections d'insuline

Abstract : Diabetes is a common chronic disease. Self-monitoring of blood glucose (SMBG) and injection of insulin is an indispensable part of the management of the disease. Repeated execution of them can be the cause of pain that are often underestimated by caregivers. In order to better understand how general practitioners perceive, address and manage the pain, we have carried out a qualitative survey based on sixteen semi-structured interviews of practitioners based in the area of Grenoble. Doctors that were interviewed considered the injection of insulin painless and SMBG hardly painful. Few of them address the question of pain in consultations usually because of omission, lack of solutions or a less priority of those pain in front of other challenges encountered in this complex disease. This perception of practitioners is in contradiction with the feelings of patients. It is probably difficult for caregivers to evaluate the pain of others due to the differences in people's pain perception. They are however sensitive to the preconception of pain due this treatments, which often brings them to change their schedule of treatment. The practitioners that were interviewed told us their difficulties in treating diabetes and diabetic patients. Moreover doctors are influenced by their negative idea of these patients and their disease. The main aim is to encourage patients to talk about their pain for a better doctor-patient relationship and to improve treatment compliance. In order to achieve this, we need to raise the awareness of doctors and give better information on the existing products and services that are intended to help manage these patients.
