Alcool, baclofène et prescription hors AMM : une étude qualitative du choix de prescription des médecins généralistes

Abstract : Baclofen, a newcomer in the treatment of alcohol problems, appeared in nonacademic ways. Its use in this indication raises scientific questions, as its evaluation is usually considered to be incomplete, as well as statutory questions, its prescription being offlabel, waiting for a possible acknowledgment (a temporary recommendation is under study in early 2013). The first part attempts to evoke the scientist and statutory contexts of this prescription. The second part relates a study of the opinions of prescribers and nonprescribers of baclofen. 14 General Practitioners in liberal were interviewed, 8 of them prescribing off-label baclofen to patients with alcohol problems, with a qualitative approach. Perplexity, even confusions, about off-label prescription and its legal context, are quite frequent in our sample. Off-label prescription appears to be experienced as an infringement. The question about off-label prescription is presently a topical one, after public health affairs with great upshot in France, although only a few papers were published to date. In our sample, concerning baclofen, prescription or non-prescription seem to be experienced in terms of risks: risks for the patient if the physician does not prescribe, and risks for the physician if he prescribes. As no guidelines are presently available, it is up to the physician to assess such risks, according to his own conscience.
