ENVOL : Étude du Nerf Optique et des Voies Visuelles en IRM 3Teslas dans le glaucome

Abstract : Purpose: To evaluate optic radiations diffusion tensor imaging (DTI) parameters in glaucoma patients and controls, and to correlate Fractional Anisotropy with the severity of glaucomatous damage. Methods: Fifty patients with glaucoma and 50 age and sex-matched control subjects underwent ophthalmological exam and 3T MRI. Main outcome measures included: cup/disc ratio, automatic visual field (Octopus, Haag-Streit), Retinal Nerve Fiber Layer with Spectral Domain OCT and fractional anisotropy. Results: 99 subjects were included. In group comparison, fractional anisotropy (0.57 vs. 0.59, P<0.0009) was significantly lower in the glaucoma compared with those of control subjects. In glaucoma patients, homolateral fractional anisotropy decreased as the glaucoma severity increased using cup disc/ratio (p=0.0002), Mean Deviation (p=0.008) and RNFL (p=0.0003). Conclusions: DTI may be an useful technique for detection and evaluation of glaucomatous damage in the optic nerve. Glaucoma has to be considered as a neurodegenerative disease not strictly limited to the eye. Future studies are needed to evaluate association between glaucoma and other neurodegenerative diseases.
