Tomothérapie hélicoïdale de chordomes rachidiens : étude française multicentrique de 30 cas et revue de littérature

Abstract : BACKGROUND: Chordomas are rare, locally aggressive, primary bone cancers with a high recurrence rate after surgical resection and a known resistance to standard radiation dose regimes. Helical tomotherapy allows the delivery of high doses to tumor volumes while sparing healthy tissues. MATERIALS AND METHODS: Between 2007 and 2013, 30 patients with chordomas were treated with tomotherapy in 5 French institutions. Survival rates without local recurrence, global recurrence and without metastasis were collected. Clinical efficacy, toxicity and treatment quality were evaluated. A comparative dosimetric study was realized for proton therapy. RESULTS: Two-year survival rates without local recurrence, global recurrence and without metastasis were 69.9%, 96.7% and 76.4%, respectively. Tomotherapy treatments were well tolerated with a relative efficacy on pain relief and no grade 4 or 5 toxicities. Tomotherapy allowed the delivery of a mean dose of 68 Gy while respecting organ at risk (OAR) dose constraints, in particular spinal cord and cauda equina. The comparison with proton therapy demonstrated optimal conformity and homogeneity indices, which were comparable in the two techniques. Proton therapy allowed the reduction of minimum and mean doses to OARs and non-target tissues.
