L'artériopathie oblitérante des membres inférieurs d’origine athéromateuse en Guadeloupe : une étude descriptive de patients ayant été vus en cabinet de médecine vasculaire et au centre d’exploration cardiovasculaire du Centre Hospitalier Universitaire

Abstract : The lower limb peripheral arterial disease is a serious pathology because of its local and overall prognosis. Material and methods: We performed a prospective descriptive noninterventional study in Guadeloupe. Data for all patients who underwent doppler ultrasound of the lower limb in a vascular medical office and in the hospital of Guadeloupe ; patients were symptomatic or not, with an ABI<0,9 or >1,3, included from march to june 2014. Objectives: Identify PAD localization, risk factor associated to PAD topography and feature of PAD in Guadeloupe. Results: 268 patients were included. 227 patients had an infra popliteal localization, and respectively, 148, 185 and 115 patients had poplital, femoral and aorto topographie. More patients had an aorto iliac (16 patients [70,48; 89,52] p<0,05) and a femoral (12 patients [52,88; 67,12] p<0,05) localization associated with tabagism. More patients had an infrapopliteal localization associated with diabete (133 patients [68,9; 87,5] P<0,05). We had 136 women and 132 men, old of 73,1 ±10,8 years. PAD were known for 52% of the population. 147 patients were asymptomatic. Cardiovascular risk factor: 237 patients had High blood pressure, 170 were diabetics, 121 had a dyslipidaemia, 20 were smokers. Other localization: 37 patients had an ischemic heart disease, 48 patients had a cerebrovascular disease and 10 patients had the 3 localization. Medical treatment : 87,1% had anantiaggregation treatment, 72,8% had a statin, 72,1% had a IEC or ARAII, and 21,4% walk 3 times per week. The surgery: 11,4% had an amputation, 20% of bypass and 32,1% an endovascular treatment. Conclusion: In our population, an infrapopliteal localization is more often than a proximal localization. Distal localization is associated with diabete, and proximal localization is associated with smocking patients. Our patients are more diabetics and less smokers than in the hexagon. We could improved patient’s walking rehabilitation.
