Évolution en ambulatoire de la dénutrition des personnes âgées de plus de 70 ans, diagnostiquée au cours d’une hospitalisation dans un service de médecine polyvalente : étude prospective

Abstract : Introduction: The prevalence of undernutrition increases with age, and may cause many physical and psychological issues. This pathology has been identified post-hospitalization as a factor related to functional decline and dependence. It can significantly affect the mortality rate over 2 years, and therefore be considered a serious public health problem. Materials and Methods: Our mono-centric and prospective observational study involved undernourished patients over 70 years old returning home after a hospital stay in a multi-disciplinary medical service. The main goal of this study was to evaluate the evolution of the nutritional status of undernourished outpatients over 70. These patients were diagnosed and received nutritional advice during their stay in hospital. The secondary goal was to identify factors affecting the evolution of the nutritional status (therapeutic, medical or environmental). The main endpoint was albumin and weight. Results: Seventy-five patients were enrolled, 32 patients had severe undernutrition and 43 mild or moderate undernutrition. Over 13 weeks (+/- 2 weeks), out of all patients enrolled, 10 (13.3%) had died. Out of the 10 patients who died, 7 were initially severely undernourished, 3 were moderately undernourished. Among the patients who survived, 38 (58.5%) had followed the prescribed recommendations and had their biology control done 13 weeks after: 9 (23.7%) were still undernourished, 29 (76.3%) were not considered undernourished anymore. Conclusion: Our study showed a good evolution of the state of health once the undernutrition was diagnosed in medical department. However, it is also clear that doctors and pharmacists do not seem quite aware of the relevance of this support and counselling. It seems necessary and essential to encourage specific training as well as a better communication and exchange between dieticians, geriatric hospital teams, general practitioners and pharmacists.
