Épidémiologie des événements de santé rencontrés dans les refuges gardés de France en saison estivale

Abstract : Context: Alpine leisure-time activities are increasingly popular and attract a large public. The geographic isolation of the mountain shelters makes them ideal places of aid where health problems are concerned. The warden can therefore be considered, in spite of himself, the principal link in the rescue chain. Objective: To report on health incidents facing the mountain shelter wardens during the course of the summer season. Materials and method: A national descriptive and prospective study was carried out between the 14th of July and the 24th of August 2014. The 126 mountain shelters included were provided with a register and the warden completed a form whenever someone with a health issue required his assistance. Results: Among 306 casualties, average age 36,1 years and principally hikers, 501 occurrences were reported. 66% of individuals had as their main complaint, a problem of traumatology, essentially wounds. The traumatisms included, above all, the lower limb with mainly ankle sprains. Medical incidents (44%) were, on the whole, in the form of headaches, fainting episodes and digestive complaints. 71% of casualties needed use of the shelter’s medical supplies with the main emphasis being on material rather than medicine. Helicopter evacuation was required for 20% of casualties. In 63% of cases, the warden dealt with the health issues alone. Conclusion: These results have enabled a reference source for training programmes for non-professional first aiders and a medical kit for mountain shelters. These measures will maximise medical care of casualties and reduce unnecessary call outs of emergency services and expenditure.
