Le retentissement de l'infertilité et de l'assistance médicale à la procréation sur la sexualité des couples

Abstract : Introduction: We talk about a satisfactory sexuality when it provides a moral and physical blossoming for the two partners and when it enables reproduction. Sexuality and procreation are intimately related, however if reproduction is a characteristic of sexuality, the aim of sexuality is not procreation. We have to distinguish "sexuality as pleasure" and "sexuality as reproduction". The purpose of our study is to assess the effect of infertility and medically assisted reproduction on sexuality in couples. Materials and methods: It concerns a prospective observational bicentric study conducted in Bordeaux and Strasbourg hospitals from February to July 2013. Data have been gathered through anonymous self-questionnaires. The six criteria for analysis are as following: sex, age, infertility types, infertility source, medically assisted reproduction type and time process. Chi-2 and Fisher stastistical tests are used in this study. Results: In total, 913 questionnaires have been dispatched, among which 501 have been given back and 485 could be taped. Sex effect points out women are more affected by infertility than men and feel more the need to express themselves about this topic (98.42% versus 90.61% ; p<0.05). Youngest people succeed in keeping a higher frequency in sexual relations in spite of medical care compared to 36-years-old and more people (p<0.05). Then older couples feel more desire for others partners (p=0.02). This study has also highlighted a difference of behaviour between homologous in vitro fertilisation and heterologous in vitro fertilisation. The more procedures last a long time, the more we feel harmful effects on sexual relations. (19.72% regarding registered couples for less than on year versus 37.05% registered ones for more than three years; p<0.05). Conclusion: Medically assisted reproduction leads to a change of sexual behaviour in couples and may be felt either as a crisis, or as a source of reunification and dialogue. Medical staff plays a crucial part into sexuality issues.
