Pourquoi les soins palliatifs ne sont pas instaurés précocement ? Étude des représentations des médecins généralistes à l’égard des soins palliatifs

Abstract : To establish the palliative care from the announcement of a serious illness improves the quality of life of the patients, according to an American study appeared in 2010 in the New England Journal of Medicine. The objective of our study is to estimate if the representations of the general practitioners influence their practice of putting on the way palliative care. A preinvestigation, realized by a managed questionnaire sent to 100 general practitioners of Gironde, allowed us to show that the palliative care are established in a late way and that there is a lack of training towards these care. A managed semi questionnaire was then realized with 7 of these voluntary general practitioners. A qualitative and descriptive analysis of these conversations was then realized. The results show that the practitioners do not make appeal to a health care team palliatives during the announcement of the diagnosis of incurability of a serious illness. The term “palliative care” is little used by the general practitioners. They are afraid of the effect which it could cause at their patient’s, even in very advanced stage. The connotation still negative of the palliative word, and all the representations which he sends back are a brake in the premature grip in care palliative. The doctor would have to can carry a look, even criticize his own representations. It is now a question of not separating any more curative and palliative care, but of associating them in a concept of unique care.
