La démographie médicale dans le 15e arrondissement de Paris et son évolution à deux et cinq ans

Abstract : The medical demography in the 15th district of Paris and its evolution in two and in five years.
Introduction: The national and Parisian demographic situation is worrying. Unfortunately the existing data is not sufficient for an effective assessment of the care offer in the primary sector and its evolution by 2018.
The objective of our study is to determine the actual primary care offer in the 15th district and the foreseeable evolution in two and in five years for the following specialities: general medicine, paediatrics and gynaecology.
Materials and method: A declarative investigation by questionnaire with reminders to all general practitioners, paediatricians and gynaecologist of the 15th district from June to October 2013.
Results: 223 general practitioners, 53 gynaecologists and 28 paediatricians were included with response rates of respectively 41,7 %, 17 % and 17,9 %. 68 % of the general practitioners were over 50 years old. 27 % refused regularly patients for consultation and for house calls. 65 % of the general practitioners had a sector 1 activity, 29 % a sector 2 activity and 5 % were off-convention. 18 % of responding participants had an exclusive particular mode of practice and did not participate in the primary care offer. By 2018, 22 % of the general practitioners and 50 % of the gynecologists of the 15th district will not practice anymore in Paris. 80 % of the paediatricians thought of staying in Paris for the next five years.
Conclusion: The aging, the feminization and the reduction in the care offer is confirmed by our study in the 15th Parisian district. To maintain the care offer, 45 general practitioners would have to establish themselves in the 15th district in the next five years.
