La place du curetage partiel dans le traitement des caries profondes : une description de pratiques professionnelles au sein des chirurgiens-dentistes de Gironde

Abstract : Introduction: incomplete removal of deep caries has been shown to reduce the risks of pulp exposure. Incomplete removal can be achieved in one or two times with a temporization period. Success rates depend on the distinction between affected and infected dentin and on the placement of a tight sealing restoration. It is therefore of interest to study whether dentists perform one- or two-step incomplete excavation or complete excavation, and which criteria and methods they use. Materials and methods: after a phone solicitation, we send our questionnaire by e-mail to 333 dentists, 54 (16,2%) of which responded. Results: demographic analysis does indicate that partitioners who responded were younger, more urban, employed more assistant and were more likely to operate in a dental corporation than the national average. 65% of dentists considered only complete excavation. The other practitioners chose one step technique for 16% of them and step-wise excavation for 84%. 31% of dentists who chose incomplete removal did respect bibliographic's recommendation. We notice that dentists who performed incomplete removal were older than the rest of our sample. To treat an exposed pulp, 61% of dentists considered direct pulp capping. 52% of practitioners received information regarding incomplete excavation and 74% agreed to be trained to these techniques. Discussion: Like most of similar surveys, our study reveals that most of the interrogated dentists do not consider incomplete caries removal as a reliable technique. Nevertheless, most of the responders were ready to received a formation to this technique which could be included to academic or continuous vocational training programs.
