Élaboration d'une échelle mesurant la communication médecin-malade : validation et description de ses déterminants à partir d'une étude réalisée aux urgences

Abstract : Background: The Physician-Patient Communication (PPC) is fundamental for the efficacy of health care. It may influence the patient satisfaction and patient adhesion. In the medical literature, no consensus tool was found to evaluate the quality of the PPC. Hence, the aim of this thesis was to develop a scale to assess the PPC during a consultation and to describe its determinants. Material and methods: A literature search identified 20 validated scales from which 15 relevant items were selected by a multidisciplinary team: scale PPC-15. The determinants and intrinsic properties of this questionnaire were investigated and validated through a study on 150 patients with an acute pathology from two emergency departments. Results: The PPC scale has a good internal consistency with a Cronbach's alpha of 0.89. The determinants of a good PPC are the level of information given by the physician (lifestyle and nutrition advices, re-consultation criteria) (p-value = 0.01) and whether the patient works in the healthcare domain (p-value = 0.04). Conclusion: This generic tool could allow evaluating the impact of an intervention in the course of a consultation. Furthermore, it could be used as an indicator of quality of care. In order to validate the extrinsic properties, the PPC-15 scale will have to be tested in other conditions. Information is the main determinant of the communication quality and has a multiplying effect on the score of the PPC. This score has also an impact on the global adhesion of the patient. One of the improvements of the communication resides in the quality of the information delivered during a consultation.
