Étude descriptive des motifs de consultation aux urgences pédiatriques pour une exacerbation d’asthme légère à modérée chez des enfants déjà suivis en ville pour l’asthme

Abstract : Introduction : Asthma is the most common chronic disease in children. Despite efficient treatments and complete recommendations, visits to emergency departments (ED) for asthma are still important and don’t decrease.
Our objectives were to describe the reasons of visiting ED for followed asthmatic children with a light or moderate asthma exacerbation, and to evaluate families’s representations of asthma and its treatment.
Method: it was a prospective, descriptive, bicenter study, including asthmatic patients of 2 years to 15 years, followed for their asthma, visiting ED for a light or moderate asthma exacerbation.
Results: 53 patients were included, median age was 6,5 years, 30 patients (56,6%) had a written action plan. The most cited reason for having visited ED was anxiety (17 families, 32,1%) followed by inefficiency of the treatment started at home, quoted by 16 families (30,2%). 35 patients (66%) answered that they knew signs of a severe asthma exacerbation and 21 patients (60%) among them didn’t quote at least one sign of a severe attack. 36 families (67,9%) knew how to define an asthma exacerbation.
Conclusion: Our study suggests that concern and anxiety were dominant in those families followed for asthma who had good notions of the disease and the treatments. Furthermore it highlighted an overrating of the severity of an exacerbation. A deepening of the therapeutic education focus of the crisis management seems to be inevitable, and a written support proposed to the general practitioners could help to improve self-management of asthma and its exacerbation by the families.
