Étude de la population adressée aux urgences pédiatriques par leur médecin traitant pour exacerbation asthmatique

Abstract : Study of children directed by their General Practitioner to the Pediatric Emergencies for Asthmatic Exacerbation. Asthmatic exacerbation is a frequent reason for emergency department and general practitioner consultation. Its treatment is well-determinate. We compared retrospectively the children directed by their general practitioner to the Pediatric Emergencies of Trousseau Hospital (Paris, France) for Asthmatic Exacerbation to those consulting spontaneously for the same reason. We studied: the presence and treatment of the underlying asthma, the exacerbation’s severity, the treatment set-up in the Emergency department and their evolution. 54 patients directed by their General Practitioner and 219 consulting spontaneously were included. The 2 populations presented comparable age (mean: 4 years old) and a clear male predominance (2 boys for 1 girl). The part of known asthmatic children is the same in both groups. A more important part of children directed by their general practioner don’t have neither controller nor reliever treatment. Systemic Corticosteroid therapy is not systematical before directing the patient to Emergency department. And Its administration in the emergency department is still belated (39.2% more than 1 hour after admission). The exacerbation’s severity, clinically estimated on emergency’s arrival is also equal in both groups, as is the emergency set-up treatment. Yet, we notice that a significative higher number of the children directed by the general practitioner remain oxygen-dependent and require hospitalization.
