Hospitalisations pour "maintien au domicile difficile" des personnes âgées dépendantes en court séjour gériatrique au CHCB : caractéristiques médico-sociales de ces patients et facteurs prédictifs de non-retour au domicile

Abstract : Objective. To describe the homebound elderly who are hospitalized in an acute geriatric unit for reason of “difficult home-based care” and to define predictors of institutionalization after hospitalization. Methods. We conducted a prospective observational study in two acute care units and included participants older than 75 years, with ADL ≤ 3, arriving from home, who are hospitalized for “difficult home-based care”, admitted between October 2013 and March 2014. Results. Among 57 patients, 20 (35.1%) were discharged and were compared with 27 (47.4%) participants admitted to an institution. The intend to place (p<0.001), the lack of APA before hospitalization (p = 0.04) and thyroid disease (p = 0.05) were independent predictors of institutionalization for elderly hospitalized for “difficult home-based care”. Conclusion. Efforts are still needed on frequent reassessment of autonomy and the aid plan, early support for caregivers, information, screening for thyroid dysfunction and anticipating crisis situations.
