La prise en charge préventive des complications thromboemboliques veineuses pendant la grossesse et le post-partum : évolution sur dix ans en Basse-Normandie

Abstract : The main objective of this study was to know whether or not the protocol that was set up at the University Hospital of Caen and Lower Normandy region about venous thromboembolism (VTE) during pregnancy and the postpartum period, has allowed an earlier and more complete identification of the risk factors. The purpose was also to determine whether or not the introduction of a preventive treatment was more appropriate since the introduction of this protocol in 2009. The comparative retrospective study was carried out in eleven maternities of Lower Normandy from the 1st of January 2004 to the 31st of December 2014. We have included 30 women who developed VTE episode during pregnancy or the postpartum period, from 2004 to 2009 and 48 women where VTE occurred between 2010 and 2014. All these patients have consulted Miss Le Querrec (biologist specialized in the identification of hereditary or acquired biological risk factors), following the appearance of VTE during pregnancy or postpartum period. Since the introduction of the protocol, VTE risk factors were better identified for women at major and high risks, and led to the setting up of a preventive treatment with anticoagulants. Women at moderate VTE risk (including at least 3 clinical risk factors) were strongly encouraged to receive a preventive treatment postpartum, according to the recommendations. In practice, this preventive measure was only made in 25 % of the cases. VTE is a multifactorial disease, and therefore, the identification of the risk factors is complex. An informative notice for health professionals help for health professionals could be put in place in order to improve the patients care.
