Un taux élevé de facteur II est-il un facteur de risque de récidive thromboembolique veineuse ?

Abstract : Venous thromboembolism is the third most common cardiovascular disease with an annual incidence of 0.5 to 2 cases per thousand inhabitants in Caucasian population. Recommendations exist in the research of risk factors for thrombosis recurrence with a particular interest for anticoagulation treatments modulation. Even though its role as a risk factor for a first episode is known since 1996 thanks to Poort et al.’s study, it has never been evaluated for recurrent thromboembolism. Our aim was to determinate whether a high level of Factor II was a risk factor for recurrent thromboembolism. We established a list of 6540 adults who underwent thrombophilia screening blood tests in Bordeaux University Hospital between July 1st, 2009 and April 28th, 2014. This assessment included a systematic dosage of factor II. 964 patients were included and randomized into strata according to their factor II level. They were all contacted by mail and sometimes by phone calls between August and November 2014. Types of thrombotic events, their circumstances and the existence of a recurrence were informed. 335 patients participated, 57 declared a single episode and 48 at least one recurrence. In univariate analysis, there was no significant relationship between factor II levels and recurrent thromboembolism (OR = 1.00, 95% CI [0.98 to 1.01], p = 0.62). Results were similar and not significant after adjustment on other variables. We didn’t show a significant relationship between factor II levels and recurrent thromboembolism. Our study, the first on this subject, should be complemented by other more powerful studies.
