Traitements antalgiques, anxiolytiques et sédatifs en phase palliative terminale en pédiatrie : étude rétrospective dans les services publics de pédiatrie et de néonatologie en Aquitaine entre 2008 et 2012

Abstract : Introduction : Deaths in pediatric units are seldom events. The purpose of this study is to identify the antalgic, anxiolytic, and sedative drugs used for the terminal palliative phase in the pediatric and neonatologic public units in Aquitaine: the indication, the way of administration, symptoms assessment, records of the informations given to the children and their families. Method : descriptive, retrospective multicentric study on children's records from 0 to 18 years old, who died in pediatric and neonatologic public unit in Aquitaine between 01/01/2008 and 31/12/2012. Results : 91 patients were included, 58 in the University Hospital Center of Bordeaux, 33 in local hospitals. Mediane age was 5,7 years [0-18,6 years]. The most frequent causes of death were cancer (37,4%), neurologic affections (31,9%) and perinatal affections (19,8%). Drugs given were morphine (60,4%), midazolam (41,8%) and clorazepate (14,3%). No drug was added for 24,2% of the patients, mostly in the University Hospital Center (p=0,04). Midazolam was more administered in local hospitals (p=0,02). Pain was assessed for 42,9% of the patients, mostly in local hospitals (p=0,002). Record of the treatment decision making process was found for 83,5% of the patients, with informations for the family in 82;4%, and the children in 9,9%. Conclusion : Pain and symptoms assessment could be improved for a better management of care and drugs. Regular formations of the medical and paramedical staff can help them handle these situations. Record of the informations given to the family by the medical staff should be more accurate.
