Problèmes soulevés par le suivi des pathologies chroniques en milieu rural et solutions proposées : étude européenne

Abstract : OBJECTIVES. Purpose was to find out problems encountered by European practitioners in rural areas within their practice connected with care for chronic morbidity patients, and to collect their opinions, solutions already implemented and suggestions. METHODOLOGY. Qualitative descriptive study with prospective data collection was performed from February to July 2015 by questionnaire with additive results from April 2015 WONCA Rural meeting. Survey population was European rural general practitioners (GPs). Qualitative analysis was led jointly with the Faculty of Medicine of Eylul University Turkey. RESULTS. We collected 189 answers from 28 nationalities; age of respondents was: 27-76 years old (mean 48.7). Study group consisted of: 46% female respondents (IC95 39-53) and 54% male (IC95 47-61). Pathologies most frequently related were diabetes 82% (IC95 76-87), cardiovascular diseases 81% (IC95 75-86) and psychiatric disorders 71% (IC95 65-77). Main issues reported were: - Patients non-compliance, education issue, and reiterative consultations. - Diseases owing to problems of screening, a high prevalence and the association of several pathologies with dependence and disability. - Practitioners with mainly a lack of time for GPs and a sense of poor recognition of their involvement. - Health system due to bureaucracy, poverty / costs problems and the access to rehabilitation programs. - Practical difficulties linked with polymedication and distance problems. CONCLUSION. Beyond issues, some positive experiences have to be promoted and would be presented to health authorities in a position paper. However, GPs might keep vigilant on some critical points like limits on their competences.
