Le rôle du médecin généraliste dans le suivi du nourrisson de moins de 24 mois : étude menée auprès de 14 médecins généralistes exerçant en Ile-de-France et en Aquitaine

Abstract : Introduction: Currently, the follow-up of the infant of less than 24 months is distributed between general practitioners, pediatrists and practitioners of maternal and child protection centers. The medical demographic trends and the government projects gradually place the general practitioner in central position in this follow-up. The main objective of this study is to observe a diversity of the practices of the general practitioners in this follow-up. The secondary objectives are to observe the missions which they give each other, the relationships they have with the other health professionals, and to know their proposals to improve the health care for infants. Materials and Methods: 14 general practitioners of Ile-de-France and Aquitaine evoked their practices during recorded semi-structured interviews. All interviews were transcribed and then, analysed with the software N’Vivo. Results: The general practitioners consider that the follow-up of the infants is one of their missions. Available, accessible economically, they integrate this follow-up in a total family care. Screening, information, mentoring, and prevention are the main objectives of this follow-up. Practitioners are satisfied with this care and little of them want to change it. However, each practitioner has different practices. There are gaps in the calendar of followup, certain examination, like visual and auditive screening, are summary, and certain information given to the parents are not really good. Conclusion: The general practitioners feel invested a significant role in the follow-up of the infant. But the practices are variable and the existing gaps must make reconsider the organization of the system of care in order to optimize the infant’s care.
