Insertion des médecins traitants dans le suivi à long terme des enfants et adolescents guéris de cancer : évaluation de la faisabilité du PPAC (Programme Personnalisé de l’Après-Cancer) en région Aquitaine par une enquête descriptive réalisée auprès des médecins traitants impliqués

Abstract : In France, the 5 years current survival rate for paediatric cancers is 82%, hence there is an increasing number of childhood cancer suvivors. During their life, these patients will be exposed to a higt risk of morbidity and mortality justifying the set-up of a medical long-term follow-up by primary care physicians, in collaboration with reference paediatric oncologists. In 2014, after two years of PPAC’s assessement in the Aquitaine region in a cohort of 128 children and adolescents initialy treated in the paediatric oncology department of the University Hospital of Bordeaux between 2005 and 2006, we evaluated its feasability by conducing a descriptive questionnaire survey with family physicians and paediatricians involved, in order to obtain their overall opinion on this type of follow-up. Among the 122 physicians, 84 (68,9%) adopted the PPAC. This corresponded to 89 patients (69,5%) who had a first long term consultation. Wit! h a 80.3% response rate (98/122), our survey confirmed the strong motivation (95.7%) of general practitioners of being involved in a shared-care follow-up with the hospital ; they were satisfied with this approach, responding clearly to an expectation, and 90% of them estimated that the PPAC was feasible in the long term. Overall, we showed physicians and patients’ interest for a shared follow-up, and that this type of monitoring is feasable. Thus the insertion of general practitioners in long term follow-up of survivors of childhood cancer improves the quality of supervision and potentially its duration on the long term. Generalizing this approach at the national level seems possible and desirable, the PPAC can therefore enroll in the continuity of major national cohort of paediatric cancers.
