Influence du stage chez le médecin généraliste sur l’empathie des externes

Abstract : Objective: Empathy is an indispensable skill for the medical students to learn. By getting the students to witness a positive and empathic doctor-patient relationship, is their empathy improved by clinical training in primary care? Methods: Case-control study with undergraduate medical students divided in two groups, one group in primary care clinical training and one control group in hospital training. Empathy level was assessed using the Jefferson Scale of Physician Empathy for Medical Student (JSPE-MS) before and after the three months of internship. The primary endpoint was the difference in JSPE-MS scale between the two groups post internship, after adjustment to allow for the multiple confusion factors. Results: 78 students were recruited before their clinical training, 26 in a primary care setting, and 52 in hospital. The mean of the JSPE-MS pre internship were respectively 107.5 and 102.5, and post internship were respectively 112.2 and 103. The students in primary care clinical training increased significantly their JSPE-MS score compared to the students in hospital (p=0.041), adjusted to their JSPE-MS score pre internship. Conclusion: Scores of empathy are increased after a clinical training in primary care, which should encourage the faculty to promote the family medicine clerkship to undergraduate medical students.
