Description de la prise en charge des cancers et hémopathies malignes chez les patients âgés hospitalisés en court séjour gériatrique et identification des facteurs pronostiques de décès à 18 mois

Abstract : Introduction: The main objective of this work is to determine the cancer mortality risk factors at 18 months in a population of old patients hospitalised in an acute geriatric ward. The secondary objective is to demonstrate which of the same factors influence the decision making between curative or palliative treatment. Method: A single-center retrospective study, conducted in geriatric medicine departments of the Bordeaux University Hospital of eighty years old patients for who an incident cancer was diagnosed between January 1st 2013 and December 31th, 2014 . Results: 133 patients were included in the study. At 18 months, 61% of patients had died of their cancer. Within the study population, only 20,7 % received a curative treatment. The absence of cognitive impairment (OR, 6,4; 95% CI, 1,052 to 38,803; p = 0,04), a history of cancer (OR, 3,5; 95% CI, 1,080 to 11,618; p = 0,04) and the administration of a palliative treatment (OR, 14,6; 95% CI, 2,684 to 78,997; p = 0,02) were independently associated with an increased risk of cancer mortality at 18 months. Patients with no cognitive impairment (OR, 3,6; 95% CI, 0,962 to 13,505; p = 0,057) and the absence of metastasis (OR, 4,605; 95% CI, 1,129 to 18,780; p = 0,033) were factors independently associated with a curative treatment. Conclusion: Our results underline the importance of cognitive abilities in decision making curative treatment but also in mortality. This work although retrospective comes to enhance existing knowledge in geriatric oncology and can be the starting point of prospective studies in this population.
