Encéphalite à anticorps anti-récepteur N-méthyl-D-aspartate chez l'enfant et l'adolescent

Abstract : Encephalitis with N-methyl-D-aspartate receptor antibody is a recent disease. The purpose of this study was to describe pediatric cases, to complete the French cohort and to compare our results with other studies. Thirty-seven cases were diagnosed between January the first 2013 and June the thirtieth 2015. The median age was 10.6 years old and 26 patients (70%) were female. Prodromes were seen in 43% of patients, seizures were the most common initial symptom and then the most common signs were psychiatric (92%) and cognitive (86.5%) disorders, abnormal movements (86.5%) and impaired consciousness (70%). Lumbar puncture showed an inflammatory cerebrospinal fluid in 81% of cases and allowed the diagnosis of encephalitis with a median of 24 days after the first symptom of the disease. Brain MRI was abnormal in 35% of patients, EEG were altered in all patients. A tumor was diagnosed in 5 patients (13.5%), it was an ovarian teratoma in 4 of these patients. All patients received first-line treatment and 84% of patients received second-line treatment. At the last follow up, one patient was dead, 76% of patients had a favorable evolution and 46% of patients were cured; it was not noted any relapse. The results of our study were coupled with data from the study conducted by Zekeridou et al to obtain a cohort of 73 pediatric cases of encephalitis with anti-NMDA-R antibodies diagnosed since 2007. The encephalitis antibodies NMDA-R is now well known in children and adolescents, the mechanisms behind this disease remain to be explored.
