État des lieux de la prise en charge de la bronchopneumopathie chronique obstructive en état stable et en vraie vie

Abstract : Introduction: The actual epidemiologic data about COPD are limited, especially the prevalence of this disease and the medical care’s management in real life. Objective: To describe the management of COPD in stable condition and in real life, and to compare it with the SPLF’s recommendations for good clinical practice. Methods: A prospective cohort from January 2014. 45 pulmonologists of Aquitaine and Charente-Maritime have seized web-questionnaires for evaluation of COPD patient’s demographic, clinical, and management characteristics. Results: 1391 patients were included, 179 with mild COPD (13%), 750 with moderate COPD (54%), 369 (26%) severe COPD and 93 (7%) very severe. 36% of patients were still smokers. Smoking cessation was achieved in 15% of patients with no significant difference between the stages of severity (p = 0.768). Influenza vaccination had been performed in 59% and pneumococcus vaccination in 54% of patients. Pulmonary rehabilitation was prescribed in 10% of patients (37% in the severe stages, p <0.001). Long-acting bronchodilators were prescribed in 44% of patients. 17% and 24% of mild and moderate stages were treated with inhaled corticosteroids in combination with long-acting bronchodilators. Conclusion: There are differences between COPD guidelines and routines treatments. Non-drug treatments are underutilized, particularly for mild to moderate COPD.
