Alpha-bloquant vs. antalgiques seuls en association à la lithotritie extra corporelle : résultats d’un essai prospectif randomisé

Abstract : Purpose: To evaluate the efficiency of alpha-blockers as an adjuvant treatment after ESWL for renal and ureteral calculi, on the success rate, expulsion time, analgesics consumption, pain, and complication rate. Material and methods: We conducted a prospective randomized trial between April and December 2015, including 125 ESWL sessions. Patients were randomized by blocks of 10 into two groups; with alpha-blockers (group 1) or control group (group 0). Our primary endpoint was the success rate of the session which was defined as a residual fragment under 4mm on the one month control CT-scan. Results: The success rate was 60% for group 1 and 58% for group 0 (p=0, 79). There was no significant differences between the groups concerning expulsion time, analgesics consumption (respectively p=0,56 and p=0,25). Sub group analysis according to stone location (renal or ureteral) or size (inferior or superior to 10mm) did not reveal statistical difference in success rate between the groups. Complication rate was 4% in group 1 and 9% in group 0 (p=0, 32). Conclusion: Alpha-blockers after ESWL do not increase stone clearance, nor improve expulsion time, pain and complication rate 1 month following the session.
