Connaissances sur les infections génitales à HPV et couverture vaccinale HPV des élèves d'un collège à Paris

Abstract : In France, two vaccines are available against HPV genital infections, Gardasil® since 2006 and Cervarix® since 2008. Girls HPV vaccinal coverage, which is already low, and insufficient to provide herd immunity, has begun to decrease since 2010. In order to improve it, the french High council of public health has recommanded in 2012 a revision of the age of beginning of HPV vaccination, from 14 to 11 year old girls. The aim of our study was to evaluate the level of knowledge of this new vaccinal target about HPV genital infections and their adherence. In april 2015, we have asked the pupils of a secondary school in Paris aged 10 to 16 for the boys and 11 to 16 for the girls, with a self-administered questionnaire. Amongst the 354 pupils who answered, 315 (89%) had never heard about HPV and (336) 95% have never heard of this vaccination. Five girls have declared they had received at least one shot of an anti HPV vaccine, so that the overall vaccinal coverage in our sample was 2,8%. In constrast with these results, 111(64%) of girls declared themselves in favor of being vaccinated against HPV, thirteen year old girls have had the higher rate of approval. The major barrier was lack of knowledge on the subject. The proportion of recent (within a twelve months period prior to our study) missed opportunities of HPV vaccination during a medical consultation was of 100%.Thus it seems important to increase the knowledge of pre teenagers and that general practitioners seize every opportunity to propose HPV vaccination, which girls are widely favourable to.
