Nutrition entérale à domicile dans l’anorexie mentale : à propos d’une série de cas

Abstract : Introduction: Preliminary study on the evaluation of weight restoration by using a nasogastric feeding tube begun in hospital, for ambulatory anorexia nervosa treatment Methods: A retrospective review of patients’ folders from outpatient department of eating disorders of the Pellegrin University Hospital Center, has been performed. The study includes the adults from April 2015 to January 2016, who had a low BMI (<15) and a rapid weight loss. Patients, with atypical eating disorders or with an untreated severe chronic psychiatric condition or with malnutrition due to somatic disease, were excluded. The study didn’t include also those who refused the proposed treatment. The primary endpoint was the weekly weight gain. Results: A total of 7 patients has been finally included. The relevant population included solely women with an average age of 28. One of those refused to continue the treatment. Only 6 patients were studied. The average length of stay in hospital was 39 days. During that period of time, patients gained on average 1.25 kg per week and BMI increased from 12.93 to 15.03 kg/m², value which permitted them to return home. At home, the weight gained was on average 0.84 kg per week. The mean treatment duration was 38.8 days. The BMI increased up to 16.16 kg/m². At that threshold; the nasogastric feeding tube has been removed. On the total duration of the study, 77.8 days, the weekly weight gain was 0.86 kg and the BMI increased by 3.24 kg/m². None refeeding syndrome has been observed. Conclusion: The results of the study confirmed that the ambulatory tube feeding, in anorexia nervosa, caused a regular weight gain in accordance with the international guidelines and the latest data of science. In addition it reduced the length of hospitalization, without increasing the risk of refeeding syndrome.
