TILT : collaboration médecin-pharmacien pour l'optimisation du parcours de soins des patients BPCO : étude pilote à partir de l'observatoire COLIBRI

Abstract : Cooperation between health care professional, including pharmacist, is essential to the care of COPD, Chronic Obstructive Pulmonary Disease. COLIBRI is a web platform, already used by respiratory physicians and physiotherapists to share information. The aim of this study is to assess the contribution of pharmacist physician collaboration in the care of COPD patients, using COLIBRI. This prospective, non-randomized, non-controlled pilot study was approved by an ethical committee. To be part of the study, patients had to suffer from COPD stage 3 or 4, from at least another chronic condition, to have a long term inhaled treatment and to be part of COLIBRI. A pharmaceutical interview was performed at home. COLIBRI data were available to the pharmacist and he also contacted the patient’s general practitioner and pharmacist. A report was addressed to respiratory physician containing information about patient’s compliance, inhalation technique, comorbidities, satisfaction about medications and a medication review. 8 patients were included between December 2015 and March 2016. The conformity between data from COLIBRI and gathered during the intervention was 54.5%. Sources to obtain the most medications and comorbidities were respectively pharmacists and general practitioners. Medication compliance was very good but only 44% of patients made no mistakes during inhaled medication intake. 15 drug related problems were identified. Those results show the potential of a pharmaceutical care concerning the exhaustiveness and precision of medication data and the supply of complementary information.
