Accidentologie des courses à pied hors stades dans les Pyrénées-Atlantiques en 2015, évaluation des dispositifs de secours et mise en place du concept Se’coureur

Abstract : The number of runnings outdoor (road, trail and mountain) and the number of competitors increase year after year. But the practice of this activity rapidly expanding is not without risks. Indeed, the musculo-tendinous wounds are very frequent and the occurrence of faintness, digestive and respiratory troubles are regularly reported. Our study, led over 6 months in the French department of the Pyrenees-Atlantiques listed 2.279 wounded persons (15,8%) on 14.407 competitors in the runnings outdoor. The musculo-skeletal wounds represent 95% of the pathologies met in runnings, lower limbs being the most affected (>77,5%). The knee (41,2%) and the ankle (33,6%) are the anatomical structures the most impacted by the wounds. The medical pathologies represent less than 5% of the pathologies in running. The faintness (68,9%) and the digestive troubles (28,6%) were the 2 medical pathologies most frequently identified. Our study allowed to identify the following parameters as risks factors of wounds and medical pathologies : the male sex (OR 3,54), a sunny weather (OR 1,47), a superior temperature in 20°C (OR 1,12), a moutain running (OR 1,42), a distance superior to 13,6 kilometers (OR 1,76) and a positive variation of level superior to 400 meters (OR 1,52). By our study, we were able to list important differences in the organization of the devices of technical rescues (means of communication, emergency material and evacuation of wounded persons) and human ressources (number of first aid workers, number of doctors and lack of insurance). Finally, this work allowed to analyze the launch of the device Se'Coureur, intended to strengthen the safety of the competitors in the runnings outdoor. In spite of a low sample, Se'Coureur showed a real impact on the improvment of the coverage of the wounded persons.
