Délai de prise en charge optimal, efficacité et tolérance de la cimentoplastie dans la prise en charge des fractures vertébrales ostéoporotiques de type A1 en Gériatrie (étude rétrospective descriptive menée de 2011 a 2015 au Centre hospitalier de Bayonne)

Abstract : Vertebral fracture is the most common osteoporotic fractures and is causing chronic pain, impaired quality of life and increased morbidity and mortality. Vertebroplasty is a minimally invasive technique with the main objective to reduce pain associated with this fracture. The analgesic efficacy and acceptability of this technique are still much debated in the literature. We conducted a retrospective observational study at Bayonne’s hospital from January, 2010 and December, 2015. Our aim was firstly to evaluate the short and long-term effectiveness and tolerance of cimentoplasties. On the other hand we wanted to analyze the practices at Bayonne’s Hospital concerning the indications of cimentoplasties as part treatment of osteoporotic vertebral fractures in elderly patients (≥ 75 years). A total of 72 percutaneous gestures were performed on 55 patients concerned by 117 spinal fractures. Most percutaneous procedures (56.3%) were performed over 6 weeks after the occurrence of the fracture. The VAS pain was improved by 4.9 points over 10 (p = 9.683 x 10-11) at hospital discharge and 4 points (p = 3.326 x 10-08) at 1 year. It is reported minor complications such as leaks in 49% of patients and no major complication. For 20% of patients were collected remotely refracture. Advanced age, pathological poly character or cognitive disorder were not indicated against the gesture. Only the analysis of acute comorbidities seems to have had an impact on the decision. In our local experience at Bayonne’s hospital, vertebroplasty allowed analgesic improvement in the short term and long term with good tolerance. There is nevertheless a risk of distant refractures. Vertebroplasty can be considered for all profiles of elderly patients.
