Prévalence et impact clinique des interactions médicamenteuses avec deux anticancéreux oraux en hématologie : ibrutinib et idelalisib

Abstract : Understanding of carcinogenesis has led to the development of therapies working on mechanism of tumor proliferation. Since 2001, orally administered targeted therapies development is exponential. Those therapies are involved with drug-drug interactions and may have a major impact on efficiency of the treatment: toxicity increase with associated non-compliance risks, or decrease of exposition to the drug. However recent and often accelerated availability of those drugs has not the necessary objectivity of the evaluation of potential interactions’ clinical impact. Success of treatment is at stake, both regarding public health issues and medico-economic matter, in order to take care of all variability factors part of the well completion of the treatment. Ibrutinib and idelalisib are two tyrosine kinase inhibitors available at the same time in 2014, with similar use profiles for four B cell based malignancies: chronic lymphocytic leukemia, follicular lymphoma, mantle cell lymphoma and Waldenström illness. If those drugs have radically change the treatment of those pathologies, their pharmacokinetic and pharmacodynamic profiles could have potential drug-drug interactions. Through retrospective study involving 131 patients over 3 years, we have studied prevalence of risk of drug-drug interactions in real life. The challenge was to evaluate toxicity impact and treatment efficiency. Prevalence of risk of drug-drug interaction was estimated at 44.6% for ibrutinib patients against 43,9% for idelalisib patients. Multiplicity of factors at stakes do not to highlight a significant clinical impact and encourage to go on the analysis and face them with pharmacokinetics and pharmacodynamics data.
