Description des méningites à entérovirus et facteurs associés à la durée de séjour

Abstract : Introduction: Enterovirus are the leading cause of meningitis. Usually benign, a rapid etiologic diagnose could reduce the length of hospital stays. The aim is to describe the clinical and biological manifestations and to identify factors associated with hospital stay. Methods: retrospective, monocentric study of all cases of enteroviral meningitis diagnosed by PCR over a 40 months period. Research for factors associated with length of stay is made by multivariate linear regression. Results: of 172 cases, 34.8 % had less than 3 years and 31.5 % over 18 years. Nearly 14 % of patients were admitted to intensive care. The classic triad fever-nuchal rigidity-vomiting was present in 65.5 % of more than 3 years versus only 3.2 % less than 3 years. Hoen score was below 0.1 in 151 cases (87.8%). PCT was higher than 0.5 ng / mL in 11 / 113 cases (9.7%). Antibiotic therapy was initiated in 53.8 % of all cases (median 2.8 days), antiviral therapy in 18.1 %, morphine in 4.3%. in multivariate analysis, age over 18 years (p = 0.04), organe failure (p < 0.001) and time to obtain results of enterovirus PCR (p = 0.002) were factors associated with a higher length of stay. Biological criteria (including PCT and lactates) were not associated with length of stay. Conclusion: time to obtain enterovirus PCR results is one of the most important factors in the patients’ length of stay. The rapid availability of molecular biology results in the context of viral meningitis should reduce ancillary tests and treatment and reduce the length of stay of these patients.
