Évaluer et renforcer l'estime de soi des patients accompagnés pour un changement de comportement lié à la santé : pertinence et modalités en médecine générale : une étude qualitative auprès de médecins généralistes de l'arc alpin

Résumé : Accompanying health behavior change in general practice is common and complex. In this approach, there is no specific tool to manage a person’s self-esteem despite its established link with behavior change. The main goal of this study was to make an assessment of care of the patients’ self-esteem by general practitioners during a behavior change support, related to health. We surveyed over 30 general practitioners from the French Alps through 5 focus groups between July 2016 and January 2017. The data analysis shows that self-esteem care is integrated in health behavior change support through an intuition mechanism. The aim of the practice remains the behavior change, while reinforcing self-esteem is a corollary; It includes a patient’s assessment of his entirety, his autonomy, his skills and his process of change. It requires specific elements of posture and communication. Self-esteem is evaluated through passive and unconscious reception of information coming from the patient’s speech and posture. It is influenced by the risk behavior and its nature. This practice answers a societal request, which general practitioners are hesitant to accept, due of a lack of experience. That is why it would be relevant to encourage the notion of intuition, but also increase efforts on human sciences, such as psychology in medical training.
