Évaluation médico-économique des produits de santé : état des lieux et pistes de réflexion pour préciser sa place dans les négociations de prix

Abstract : Since 2012, a cost-effectiveness analysis is required for reimbursement purposes in France for drugs and medical devices deemed to be innovative (major, substantial or moderate improvement in actual clinical benefit) and likely to have a significant impact on the statutory national health insurance expenditures (> 20 M€). However, the lack of cost-effectiveness threshold in France, which indicates society’s willingness to pay for innovation, tends to hinder its full integration in the price negotiation process. An ongoing database of incremental cost-effectiveness ratios, based on negotiated prices, could be built in order to obtain empirical reference values for decision makers. This work, conducted at The French National Authority for Health (HAS), focuses on this issue and suggests ways to optimize the use of health economic evaluation in the health product price negotiations. In particular, proving decision-makers with a tool to link the health product price and its cost-effectiveness, after price negotiation, could help transparency and enable decision-makers to progressively build a framework that takes into account incremental cost-effectiveness ratio in price negotiations.
