Influence de la perception du médecin généraliste par les adolescents sur les opportunités de dialogue autour des conduites addictives et de la sexualité

Abstract : Background : Lately, we observe a new increase of hazardous behaviours (smoking, cannabis, sexually tramsmitted diseases) at adolescence which is a very essential time about addictives and risky sexual behaviours prevention. The general practitioner (GP) is the chosen intermediary of teenagers. Does the perception they have about him modify the dialogue’s opportunities about these themes ? Methods : We conducted a prospective multicentric study on 421 tennagers between 11 and 19. They came from 4 high school and colleges in Paris suburbs. We analysed their feelings and their hopes through a multiple-choice questionnaire. Results : Teenagers consult with a parent for 78% of them which is embarrassing for 30% of those who are more than 16. They are 96% who never have spoken about addictive behaviours with their GP, 40% want their GP to take the initiative and 26% (42% of addictive substances users (ASU)) fear that he reveals informations. Mentioning these topics with their GP is normal for 40% and annoying for 34%, 71% think he can either understand or help them and 31% (50% of ASU, p < 0,05) believe he will either judge them or tell them off. They are 93% who never have spoken about sexuality with their GP and 35% wish he takes the initiative, 55% consider it is embarrassing and 30% normal, 67% think the GP can either understand or help them and 21% fear GP either judge them or tell them off. Conclusion : Talking about these themes is rare for teenagers. The GP has to increase his involvement and to adapt his consults. Trust and secret require verbalization of privacy and neutrality. Training program for GPs seems to be necessary.
