Évaluation d'un outil type "télé expertise" dermatologique en région annécienne entre novembre 2014 et juillet 2016

Abstract : In France, General Practitioners (GP) experience major difficulties in quickly getting specialized dermatological advice for their patients. At Centre-Hospitalier-Annecy-Genevois (CHANGE), this issue has been tackled by the creation of a specific mailbox since November 2014. The study aims at explaining the role and benefits of this "tele expertise" device in primary care in Annecy. It also aims at showing how it can serve as a tool for prioritizing requests for specialized consultations. The first part of the study is retrospective observational quantitative. It analyzes mails requesting dermatological advice from the CHANGE between November 2014 and July 2016. Standardized data were collected and analyzed. There were no exclusion criteria. The second part of the study is a phone survey involving sample of general practitioners who used the tool in June 2016. Three hundred and seventy-eight telemarketing advice e-mails were included and analyzed. Eighty percent of the requests came from PG. The dermatologist replied in less than twenty-four hours in 66% of the cases. He advocated that no in vitro specialized consultation should be carried out for 61.6% of the opinions. Conversely, in 31.4% of the cases, he directly scheduled a consultation in rapid delay (less than 10 days). A significant correlation between the type of pathology motivating the opinion and the measures to be taken has been drawn. The phone survey received 78% of responses. The GPs surveyed were 97% satisfied with the service. Hundred percent of them consider it an interesting continuing medical training. The proposed tele-expertise tool seems to be a good tool for dermatological regulation in the service of PG. Patient satisfaction remains to be explored. The financial valuation of such a system remains to be established.
