Étude hépatochlord : volet descriptif. Impact du chlordécone sur sur l'évolution de l'hépatite chronique active virale (B et C) et alcoolique vers la cirrhose, en Guadeloupe

Abstract : Introduction: Chlordecone, an organochlorine pollutant, used in Guadeloupe, French West Indies, from 1973 to 1993, has led to significant pollution of natural environments, with still ongoing damage. Its pro-fibrotic impact on the liver has been demonstrated in mice populations, associated with carbon tetrachloride. This study aimed to demonstrate if exposure to chlordecone acted as a potential aggravating factor in the progression of chronic hepatitis, both viral and alcohol-related, towards liver cirrhosis. Secondary endpoint was to assess studied patients’ impregnation with chlordecone and other organochlorines. Material and Methods: A case-control retrospective study was conducted in the Gastroenterology department of Pointe-à-Pitre University Hospital, Guadeloupe, French West Indies, from November 2010 to December 2015. All adult patients, with known alcohol-related or viral chronic hepatitis were included, separated into 2 groups: F0F1 group (control group with early-stage liver fibrosis) and F2F3 group (case group, with advanced liver fibrosis). Blood levels of chlordecone and other organochlorines in those 2 groups were compared. Results: Overall, 257 patients (189 men, mean age 54.4 [24-88]) were included. No statistically significant difference in chlordecone’s blood levels was demonstrated between the 2 groups, either in men (p=0.4) or women (p=0.66). Evaluation of organochlorine’s levels depending on gender and areas of residence showed higher blood levels of these agents in women, and people from Basse-Terre area. Discussion: It is not yet possible to assess if exposure to any of the studied organochlorines promotes pro-fibrotic evolution of chronic hepatitis. We have noted high blood levels in all people living in Guadeloupe, with even higher levels for women and people from specific areas. Further studies will be conducted.
