Support pressionnel non invasif chronique chez l’enfant : état des lieux et mise en place d’un suivi standardisé au CHU de Grenoble Alpes

Abstract : Chronic respiratory failure is increasingly well understood. Hypoventilation occurs when there is disequilibrium in the respiratory balance. Its treatment is the nocturnal non-invasive ventilation. Objectives: The aim of the study was to evaluate our practices in the area of home non-invasive ventilation through the development of a standardized annual pluridisciplinary consultation. Methods: This was a descriptive, prospective and monocentric study. All children who had non-invasive ventilation at home and supported in our unit the January 1, 2016 were included. Seven patients were excluded from the study (five due to tracheostomy, one due to palliative ventilation and one due to relocated) and 38 were included. The primary endpoint was the success of the ventilation defined by proper compliance (daily use and more than half of the night without a suspension period) and the normalization of gas exchanges (defined on the recordings of SpO2 and TcPCO2 under ventilation). Results: For 54% it was a success: 65% had a proper compliance and 88% had ordinary gas exchanges. Success group used non-invasive ventilation longer than the other: 1,7 years (IC 95% (0,02-3,36) p=0,048). Other relevant factors (patient age, underlying pathology, existence of side effects) were identical in the two groups. Conclusion: The success rate in this study is closed to studies reported in literature and encourages us into the development of a specific monitoring procedure with an early reassessment regarding the childs to failure of their support.
