Détermination de la longueur de travail en endodontie : méthode radiographique, localisateur d'apex isolé ou intégré au micromoteur ?

Abstract : Aim: Assessing if there is a method more effective than the other in determining the length of work (X-ray or electronic) in terms of accuracy, reproducibility, operative time and practitioner dependency. Material and Method: Realization of a clinical study in vivo comprising a total of 105 teeth, on which 3 techniques have been tested: using the X-ray method, using an electronic locator of isolated apex (Root ZX®) or using one integrated in endodontic micro motor (Endo Master®). Results: Our study has shown that the use of electronic apex locators, in particular those of the so called "last-generation", allows to limit the importance of the practitioner dependency factor inherent to the conventional method, but also saves time in a significant way as a result of the simplification of the operative gesture. The radiation dose is lower and the reliability is equal or superior to the x-ray method. Conclusion: The X-ray remains essential today in endodontics because of the information it provides about the root morphology and potential surgery problems. However, because of the complexity of the root canal system and of the superposition of the anatomical structures on the x-ray film, this technique appears to be insufficient compared to the use of electronic apex locators based solely on physical non-anatomical parameters.
