Envahissement ganglionnaire des tumeurs mammaires du quadrant inféro-externe en fonction de la profondeur : identification d’atteintes de la chaîne mammaire interne et impact thérapeutique potentiel

Abstract : Objective: The evaluation of the depth level of the tumor in the breast parenchyma is a new concept to define the lymphatic drainage. The aim of this study is to determine a distance pectoral-tumor as a risk of involvement of the internal mammary chain (IMC). Methods: The patients who had a lymphoscintigraphy between June 2012 and November 2016, with an infero-lateral tumor, were analyzed. Results: Ninety-four patients had an infero-lateral tumor. Forty-four of them have beneficiated of a histological analysis of a node retired in the IMC. Five (11,4%) had an involvement. The ROC curve determined the threshold at 9 mm. A Chi2 test between the group of 25 patients who had a tumor less than 9 mm of the pectoral plan versus the 19 others didn’t show a statistical higher risk of involvement of the IMC (p=0,09) in the first group. Conclusion: This study isn’t so powerful to show a difference. But this optimal depth distance of 9 mm should be confirmed in other study, and should be taken in consideration by the radio-oncologist physicians for the decision of IMC treatment (with the others currently known).
