Déterminants de la communication par les médecins généralistes de renseignements cliniques aux biologistes : étude qualitative par entretiens semi-dirigés auprès de 14 Médecins généralistes dans les Pyrénées Atlantiques

Abstract : Introduction: since the reform of medical biology, the role of the medical biologist is strengthened within the care pathway. Prescriptions for biological examinations should include relevant clinical features. Only few of them contain any information. Those containing the least are those of general practitioners. The aim of this study is to identify and understand the “brakes” and “engines” to communication of clinical datas toward biologists. The secondary objective is to collect representations about the work and role of the biologist in the care network. Material and method: Qualitative study by semi-directed interviews of GPs of the Pyrénées-Atlantiques, recruited by reasoned sampling with maximum variation. Results: 14 physicians were interviewed. Only one of the interviewed GPs was aware of the reform. The majority of the interviewed GPs admit that they do not know the work of the biologist. Most GPs are satisfied with the relationship with biologists. The main facilitator to deliver clinical information toward biologists are the need for the prescriptors to benefit from the specialized opinion of the biologist and the value of the information for the biologist identified by the GP. The representation of the biologist as a trusted staff, work with an identified laboratory are facilitators. The main brakes are the patient's potential disagreement, the fear of the violation of medical confidentiality, lack of perceived usefulness or lack of knowledge of the utility of communicating clinical facts. Representation of the biologist as a provider or technician is a limiting factor. Discussion: this study shows three physicians profiles: the most communicating, the occasional communicators, the least communicating. Conclusion: although the relationships between GPs and biologists are good in most of situations, increased communication between prescriber and biologist is necessary. The knowledge about the work, interests, needs and difficulties of each other is essential to improve compliance about this practice.
