Syndrome d’apnées obstructives du sommeil et troubles de l’humeur, liens bidirectionnels

Abstract : The discussion about the relationship between sleep disorders and psychiatry is not new. These two troubles are connected in many ways. More specifically, in patients with obstructive sleep apnea syndrome (OSAS) mood disorders are frequently found and conversely in patients with mood disorder a higher prevalence of OSAS is reported. The aim of this study is to highlight the two-way links between OSAS and mood disorders, to show the importance of the diagnosis and medical care of these two disorders and to evaluate the state of current clinical research on this topic. The first part is devoted to the definition of OSAS with the study of epidemiology, its clinical description and its management. In a second stage, the epidemiological, physiopathological and prognostic link is studied successively following a double perspective each time. Through a systematic approach to each aspect, the relation and impact of OSAS on mood disorders and reciprocally mood disorders on OSAS are outlined. The importance of screening OSAS in patients with mood disorders is addressed in a fifth chapter, screening tools for this disorder and their validity in patients with mood disorders are exposed. In the sixth part, a clinical case is proposed to illustrate the two-way links between OSAS and mood disorders and the challenges of diagnosis and treatment of both pathologies when they are present in the same patient. Finally, this work advances physiopathological hypotheses, offers clinical perspectives with adapted screening tools, proposes new topics for clinical research, and reveals the value of a sleep medicine approach in psychiatry and vice versa.
